Dear Parents or Guardian,
This coming September your child will be a senior in high school.
All 12th graders are required to be vaccinated for meningitis meningococcal (menACWY); one dose only if the 1st dose was received at age 16+ or older. Some students may need a third dose as the first 2 doses were given before age of 16; proof of immunization will be needed and signed by a physician or Board of Health person responsible for giving your child these shots.
Records must be brought to Ms. Bullock in room 371 or emailed to abullock@schools.nyc.gov. Failure to provide proof of immunization could result in your child being excluded from school. Please include student name and OSIS number on all medical records.
