To replace a lost or stolen ID/Program card, complete this form available on www.stuy.edu and submit to room 209. ID/Program cards will be processed and available for pick up within two days. In the meantime, a temporary ID will be given to the student. Until receipt of the permanent ID/Program card, certain privileges, such as eating lunch outside of school, will be suspended.

Students may fill out and print the form from online and then drop it off in room 209. Their ID in will be ready for pickup in Room 209 by paying the $3 fee.
