MANDATED FOR ALL BLENDED STUDENTS - CONSENT to Covid-19 Testing -- EASY instructions for doing so in your NYCSA Account

Stuyyvesant, and all High Schools reopen for blended learning on March 22, 2021.

Consent to COVID testing is mandated for all students and staff members.

If you have aNYC Schools Account (NYCSA), you can complete the consent form electronically through the web application. If you revoke consent for testing through NYCSA, please notify Ms. Ingram at ringram3@schools.nyc.gov or Ms. DiCarlo at adicarlo@schools.nyc.gov..

If you don't have a New York City Schools Account? Get one today by emailing Ms. Ingram for an activation code. This is a parent account. Infroamtion will not be shared with students. Each parent has their own individual code.

Instructions on how to consent in NYCSA:

  • Log in to your NYC Schools account at mystudent.nyc
  • Select your student.
  • Navigate to Manage Account.
  • Click on Consent Forms
  • On the COVID-19 Consent Form you will see a list of your students
  • Choose to Consent or Deny to COVID-19 Testing.
  • There is additional information there to read.
  • Check the box to agree and consent and choose submit.
  • It will say Updated online which tells you the school has been updated;
  • f you give us a paper version it will say “updated at school”. Paper consent forms are available here.

After your child is tested -- Get Your Child’s COVID-19 Test Results

Schools offering in-person teaching and learning are conducting weekly random testing of students and staff for COVID-19.  If a student is tested, parents should hear results via text, email, or phone call within two to three days.
