Remember, 10% of unvaccinated students whose parent/guardian consent will be tested bi-weekly. You can provide proof of vaccination for your child by starting at your NYC Schools Account. You’ll need your child’s Student DOE email = xxxxx@nycstudents.net and their password. If your child does not remember their password or needs it reset they can use this form. It must be the student to request a password reset.
