You can also read this family letteras well. As a reminder, families can also submit the consent form using the NYC Schools Account (NYCSA). Remember, 10% of unvaccinated students whose parent/guardian consent will be tested bi-weekly. You can provide proof of vaccination for your child by starting at your NYC Schools Account. You will need the followingPhoto file of vaccination card (save the file to your device and attach it when prompted, either jpg or png format)

Go to: vaccine.schools.nyc and click "sign-in” You'll be directed to a different DOE site to login.
You’ll need your child’s Student DOE email = xxxxx@nycstudents.net and their password. If your child does not remember their password or needs it reset they can use this form. It must be the student to request a password reset.
