Submit your "Military Recruiting Opt-Out" Form by 10/23/26

In accordance with Chancellor's Regulation A-825, please see the military recruiting opt-out letter. Opt-out forms were included in the Camp Stuy parent orientation digital folderfor incoming families. Parents of students who are younger than 18 years of age can opt out of releasing information to military recruiters who request this information. Students have the right to make this decision once they turn 18.

You have two ways of submitting this form if you wish:

• Parents/guardians can use their New York City Schools (NYCSA) Account to complete the opt-out form using the Forms Module. This option eliminates the need for manual data-entry by staff members, as NYCSA opt-out data will be automatically migrated. For information on creating NYCSA accounts, please email Ms. Ingram at ringram3@schools.yc.gov.
• You may fill out and print the form and send back with your child to room 103.

The final deadline to collect completed opt-out forms is October 23. No entries may be entered after October 30.
