All Students received an enlarged Blue Emergency Contact Card this year. We are asking EVERY family to fill out this card and have students RETURN IT AS SOON AS POSSIBLE to the collection box at Room 103 or the Nurse in room 371, UNFOLDED. It is important that the school have accurate emergency contact information for every student. Your cooperation is appreciated. If you didn’t receive one, extras are available in Room 103 or Room 222 or 371. You MUST provide the nurse with a hard copy signed copy of the blue card. If you send me a digital copy, it will be printed and sent home with your child for a signature to return. Lost or Stolen ID form Students who need a replacement ID may fill out this form and click submit for it to be sent directly for printing. Students may pick up their new ID when they bring payment of $3 to room 209 the following day.
