<p>To <strong>replace a lost or stolen ID/Program card</strong>, complete this form available on <a href="http://www.stuy.edu">www.stuy.edu</a> and submit to room 209. ID/Program cards will be processed and available for pick up within two days. In the meantime, a temporary ID will be given to the student. Until receipt of the permanent ID/Program card, certain privileges, such as eating lunch outside of school, will be suspended.<br/>Please refer to the official school ID/Program Card Policy for further information.<br/>Students may fill out and <a href="https://stuy.enschool.org/ourpages/auto/2019/11/24/52430514/ID_card_application.pdf">print the form from online</a> and then drop it off in room 209. Their ID in will be ready for pickup in Room 209 by paying the $3 fee.</p>
