<p><strong>Girls Swimming and Diving Tryouts 2019 </strong>will take place <strong>starting on August 26</strong><sup>th</sup> in the Stuyvesant pool. Please be at the pool by 8AM.<br/>Things to bring:<br/>Required PSAL forms <br/>Bathing suit/swim cap/goggles/towel/flip flops<br/>Water/Sports drink<br/>Inhaler (if you carry one)<br/><a href="https://stuy.entest.org/2019_PSAL_DOH_PPE_2019.pdf">The Interscholastic Sports Medical Form</a> and the <a href="https://stuy.enschool.org/athletics/forms/2012_psal_interscholastic_athletics_parental_consent_form.pdf">Parental Consent Form</a> are mandatory and you must bring both with you when you arrive.</p>
<p>-The <a href="https://stuy.entest.org/2019_PSAL_DOH_PPE_2019.pdf">Interscholastic Sports Medical Form</a> must be fully completed by you, a parent/guardian, and a doctor. The doctor must complete all of the requested information. FYI: Your doctor does not need to attach any other medical history or immunization forms.<br/>-The <a href="https://stuy.enschool.org/athletics/forms/2012_psal_interscholastic_athletics_parental_consent_form.pdf">Parental Consent Form</a> must be read and signed by you and a parent/guardian.
