In compliance with the New York State Education Department’s (NYSED) request for information from families on student access to devices and internet connectivity, the DOE has added the NYSED’s Digital Equity Family Questionnaire for families to complete through their New York City School Accounts (NYCSA); the questionnaire is available in all DOE-supported languages.
Please read this notification letter and fill out the survey no later than December 1.

If you need assistance with creating your NYCSA account, please contact Dina Ingram at ringram3@schools.nyc.gov.. For parents without NYCSA accounts, who do not want to obtain one, a paper version of the questionnaire is available on the Digital Equity Family Questionnaire webpage. If you use a paper version, please return with your child to room 273C to Ms. Ingram so that and staff will be able enter families’ responses received.
